Cognitive stimulation: what it is, who it is for and how it works

Spain is ageing. According to the National Institute of Statistics, more than 20% of Spain’s population is already over 65, and the figure keeps rising. With ageing comes a quiet challenge: the decline of cognitive functions. Memory problems, difficulty sustaining attention, confusion when planning everyday tasks… These are signs that millions of families recognise, but few know how to deal with.
The good news is that the brain is not a machine that simply wears out beyond hope. The scientific evidence of recent decades has shown that, with the right stimulation, mental abilities can be maintained and even improved at any age. And this is where cognitive stimulation enters the scene — a set of techniques designed precisely for that: to keep the brain active, functional and protected.
In this article we explain what cognitive stimulation is exactly, who it is aimed at, what benefits it brings and which techniques exist today — from classic exercises to the most advanced neurostimulation technologies.
What is cognitive stimulation?
Cognitive stimulation is a structured set of activities and techniques whose aim is to activate, maintain or improve mental functions: memory, attention, orientation, language, reasoning and the ability to plan. It is not about occasionally doing crosswords or playing sudoku for fun — although both help. Cognitive stimulation is a systematic intervention with clear goals and, in a clinical context, under the supervision of specialists.
It is important to distinguish it from the popular notion of “brain training”. While brain training usually focuses on improving performance on a specific task — for example, solving puzzles faster — cognitive stimulation aims at something broader: activating the neural networks that support overall mental functioning, so that the benefit transfers to the patient’s daily life.
Who is it aimed at? Mainly three groups:

Healthy older adults who want to prevent age-related cognitive decline.

Patients with mild cognitive impairment (MCI) — an intermediate phase between normal ageing and dementia, where early intervention is especially effective.

People diagnosed with Alzheimer’s disease or another dementia in the early or moderate stage — with the aim of slowing progression and preserving autonomy for as long as possible.
What are cognitive functions and why do they decline?
To understand cognitive stimulation, you first have to understand what cognitive functions are exactly and why they can weaken.
Memory, attention, orientation, language, executive functions
Cognitive functions are the mental processes that allow us to interact with the world. A person’s cognitive ability depends on how these functions work together:

Memory: The ability to store, retain and retrieve information. It includes short-term memory (remembering a phone number you have just heard), long-term memory (childhood memories) and working memory (keeping information “active” while you carry out a task).

Attention: The ability to focus on a relevant stimulus and ignore distractions. It is the gateway to all the other functions: without attention there is no learning and no memory.

Orientation: Knowing where you are, what day it is, who you are. It seems elementary, but it is one of the first functions to decline in dementia.

Language: Understanding and producing words, building sentences, holding a coherent conversation.

Executive functions: The ability to plan, organise, make decisions, solve problems and control impulses. These are the brain’s most “complex” functions, and they are located mainly in the frontal lobe.
Causes of decline: age, illness, stress, lack of activity
Cognitive decline is not inevitable and does not have a single cause. The main factors that accelerate it are:

Natural ageing: After the age of 50, the brain gradually loses volume and the speed of information processing decreases. Even so, the intensity of this process varies greatly from one person to another.

Neurodegenerative diseases: Alzheimer’s disease, Parkinson’s disease, dementia with Lewy bodies and other illnesses that progressively damage brain tissue.

Cardiovascular factors: Hypertension, diabetes, high cholesterol and a sedentary lifestyle affect cerebral blood flow and, with it, cognitive performance.

Chronic stress and depression: Persistently elevated cortisol damages the neurons of the hippocampus, a key region for memory.

Lack of mental and social stimulation: A brain that is not used weakens. Social isolation and intellectual passivity are risk factors just as real as the biological ones.
The benefits of cognitive stimulation
The question many people ask is: does it really work? The scientific evidence accumulated over the last two decades says yes — and quite substantially.
Improvement in memory and attention. Cognitive stimulation programmes for older adults have shown measurable improvements in episodic memory, sustained attention and information-processing speed. And not only in the laboratory: these improvements are reflected in the patient’s everyday life — remembering appointments, following the thread of a conversation, managing their own medication.
Slowing of decline in Alzheimer’s and dementias. According to several meta-analyses, regularly applied cognitive stimulation in patients with mild and moderate Alzheimer’s disease can slow the progression of decline. It does not cure the disease, but it widens the window of time during which the patient preserves autonomy and quality of life.
Effect on mood. An often underestimated benefit: cognitive stimulation reduces depressive symptoms and improves self-esteem. When a person notices that they can remember better, that they take an active part in activities and that their effort produces results, their emotional state improves. This is especially important in older adults, where depression and cognitive decline tend to feed each other.
More independence for longer. The ultimate goal is not a score on a neuropsychological test, but the person’s ability to carry out everyday activities on their own for as long as possible — dressing, cooking, going out, maintaining social ties.
Cognitive stimulation techniques and exercises
There are many approaches, and the ideal is to combine several of them, adapting them to each person’s profile and needs.
Memory exercises for older adults: workbooks and cards
Memory exercises remain a basic tool. Cognitive stimulation cards — structured workbooks with exercises in memorisation, association, categorisation and sequencing — offer an accessible way to train memory at home. There are specific sets for different levels of impairment, from basic cognitive stimulation exercises to advanced programmes.
Consistency matters: short daily sessions (20–30 minutes) are more effective than long, occasional ones. And it is always preferable for the exercises to be adapted to the person’s real level, to avoid frustration.
Cognitive games and digital apps
Technology has considerably broadened the possibilities. Memory games for older adults — both in board format and in tablet apps — make it possible to train attention, reaction speed, logical thinking and memory in an enjoyable and motivating way.
Digital apps have an added advantage: they adjust automatically to the user’s level, gradually increasing the difficulty. This keeps the challenge alive without causing frustration. Even so, do not fall into the trap of believing that any game “stimulates the brain”: what is scientifically grounded are programmes specifically designed to activate particular cognitive functions.
Group activities and occupational therapy
Cognitive stimulation does not have to be a solitary activity. Group workshops — reminiscence, discussion, crafts, therapeutic cooking, music therapy — combine cognitive work with social interaction, a key protective factor against decline. Occupational therapy, in turn, embeds cognitive stimulation in real functional activities: doing the shopping, planning a route, organising a schedule.
Cognitive stimulation and technology: beyond exercises
Exercises and workshops are effective, but science has gone further. In recent years, technologies have been developed that act directly on the brain to strengthen its natural mechanisms of repair and plasticity.
Non-invasive brain stimulation (TPS)
One of the most promising lines is transcranial pulse stimulation, known as TPS (Transcranial Pulse Stimulation). Unlike cognitive exercises, which work “from the outside” by presenting tasks the brain has to solve, TPS acts from the inside, directly stimulating specific brain regions with focused, low-energy acoustic pulses.
How does it work? The device emits acoustic waves that precisely reach the brain areas chosen by the specialist. This stimulation triggers a process called neoangiogenesis — the formation of new blood vessels — which improves cerebral blood flow, optimises communication between neurons and, according to the available clinical data, helps raise levels of serotonin and dopamine.
The results are significant: according to the studies carried out, as early as three months after treatment patients experience measurable improvements in cognitive functions such as attention, orientation and memory.

At the first signs — If you start to notice frequent forgetfulness, difficulty concentrating, problems finding words or occasional disorientation, do not dismiss it as “just age”. See a neurologist. Mild cognitive impairment, detected in time, has a far better prognosis than when it is diagnosed once the disease is already advanced.
With a dementia diagnosis — Cognitive stimulation is one of the non-pharmacological techniques with the strongest evidence base in Alzheimer’s disease and other dementias. But its effectiveness is considerably higher in the early stages. Every month of delay in starting treatment is lost time.
